That’s a matter of debate.
Some advocate for down, in an effort to better stretch the dorsal ligament.
Some favor straight out, so the base has a firm placement against the pubis.
Some favor a natural anatomic approach (whichever way it naturally goes).
I personally think variety is good, so stretch is achieved from differing angles. I change angles throughout a session; as one area fatigues, I focus elsewhere. But I have had “great stretches” pointing straight down at my feet while laying flat on my bed, I must admit.
You gotta experiment to find out what feels right for you.
