How does one know they have it? Does it hurt? Even I have had not-painful, but raging hard achy erections in my sleep, but surely that can’t be it?

Priapism is a condition where a man has an erection lasting more than 4 hours and usually needs medical attention to relieve it. I doubt if your nocturnal erections are Priapism.

Yeah, I’m nutty. But I’d like to know if you could have a priapism event (drug related or not) and have no pain?

I’m guessing here, but probably not. I would think that an erection lasting over 4 hours would hurt in some manner.

Do a Google search on “painless priapism” and you will get results. Generally painless priaspism is high-flow priapism, and is frequently associated with blunt perineal trauma.

If it’s achy, how can you call it non-painful?

Because it doesn’t really ‘hurt’, I just feel an ache, similar to what you feel when you have a raging hard on.

Yeah high flow usually comes from physical damage. What about a case of painless-lowflow priapism?

I don’t think that would happen. I suspect that high flow priapism may not hurt because the high flow results in good oxygenation. Low flow priapism would mean poor oxygenation and pain.
