There are four basic types of hanger:

1) noose - out of favor and considered obsolete

2) compression - Bib and copies

3) bell and sleeve - your standard “vacuum hanger”

4) bell and gasket - the Size Doctor and copies

The bell and sleeve hangers do look difficult, and they *are* harder to put on if you have a short flaccid. However, once you get the trick they’re very easy.

Most instructions tell you to “roll the sleeve up over the bell.” Unless your sleeve is very short and/or thin, that doesn’t work well. But you can *fold* the sleeve two or three times and get it up and out of the way. You might have to cut it shorter to make it work. Do it a quarter inch at a time until you can get it to fold over the bell. You can also put the sleeve on further over the bell, as long as you can still get the pump to it.

Put your glans in the bell and unfold the first section of the sleeve. If you have a very short flaccid, as I do, you may have to work up a partial erection. It only has to be enough to do the first unfold.

Apply the mouth tube or pump and pull the air out of the bell.

You can then grasp the bell, or hook it to something, and pull your flaccid out a few inches. Then do the final unfold(s). The open end of the sleeve may get folded under in spots; adjust everything straight.

You’re now in. Ta daaa! Hook it to your harness or strap and you’re extending.

The simplest way I’ve found to remove the extender is to fold/roll it off the bell. Once you lose vacuum it’s easy. In my case, I can just push the glans down and through the sleeve and it’s off.

Most vacuum extenders come with 20mm sleeves. 18mm and 25mm sleeves are available. The 20mm sleeves are stretchy enough to handle almost any sizes shaft, but I found the 25mm was much easier to get on.

In a thread a few years ago, some guys said they dispensed with the sleeve and just rolled their foreskin up over the bell and taped it. I don’t have a foreskin, so I don’t know how this works from personal experience, but if it works for you, it would eliminate the whole sleeve hassle.
