Uncut extension without foreskin retraction

I have a case of persistent Sclerosing lymphangitis and tried extending using Penismaster Pro. I had to retract my foreskin every before going into the head piece. This made sclerosing lymphangitis far worse.

Wonder if anyone has a solution for using penis extender without retracting the foreskin. Maybe wearing a silicon sleeve over glans and then inserting it into the head piece?
