Has anyone observed if he had better gains by extending UP or DOWN?
In my case the period I was extending UP I had gained some girth.

And do you have any recommendations of what to do about the digging of the base of the extender on the dorsal vein when extending UP?

Due to the nature of the extender device (assuming we’re thinking of the same device), there is only a small area where the direction of the bend would differ, if I’m looking at this right. And that area is the base, which would be bent at a different angle.

Bending up would block the dorsal veins, causing the internal pressure to rise, but also reducing blood flow. It could also impact the dorsal nerves.

I’ve never observed gains from an extender (the next few days I observe a shortening of my erect length), but I always end up putting it on the highest tension possible.
