Where to put hanger on

Hi,

Where do you guys put the hanger on exactly? Is that right behind the glance? Or some where on the mid of the shaft?
I usually do it right behind the glance.

I need to know this asap, because I see some dark spots on my glance. Think the blood vessels are stressed beyond their limits.
