There seems to be lots of reasons why this is a good idea. If the shaft is not low in the head well then hanging can be quite unpleasant, as the hanger will tilt when its on (it will tilt slightly no matter what you do), if you look at tom hubbards page there’s a picture of this, the link is around somewhere. The tilt places slightly more pressure on the area below the glans, and the glans itself (the ‘ridge’ area). This is not a problem if the shaft placement in the hanger is low, but if its not the tilt of the hanger will be increased, putting more pressure on this sensitive area.
Also, the further down in the well the shaft is, the better grip the hanger will have on the shaft.
Its really hard to explain, it will make sense when you get it. If im wrong about this, I hope someone will correct me. But this is how it works for me. I would re-read the bibhanger site too.
Tightener,
The main tightener is a long screw with a wingnut on it attached to one side of the hanger, the other side has a groove where the screw goes in. This is a great idea because you can change the tightness easily during hanging, and you will need to, too. The old hanger I had used a hose clamp thing that you needed a screwdriver to loosen.

Yeah im kidding. I was baffled by it at first, it took a good few weeks and several clock analogies to get it right (dont ask).
