Not really sure as I never really needed it, but if you set up your hanger in a correct fashion and skin is being stretched, you still need skin stretch. You won’t need as much weight for skin stretch either, probably longer hang time and lighter weights would be better for that (for safety).

Basically yes. Slight changes in angle can hit different sets of tissues somewhat, but as long as you use the same one every time or close to it, you’ll be fine.

Whatever is most comfortable. Sit up straight though, because if you’re leaning forwards or backwards you’ll change the angle.
