What I do is I first attach the hanger 1” below my glans. I then pull it forwards to less than 0.5” below my glans and tighten the hanger stepwise. I give the hanger some tugs which loosens the tissue so I can tighten it some more. After applying the weight I tighten it just a bit more. This procedure keeps my hanger tight enough for the first 5mins or so after which I tighten it some more.

If this won’t work for you you can try to push a little bit of blood towards the end of the cc’s so the ‘stopper’ is bigger.
