Toe is basically the adjustment of the nuts near the bib hinge that determine the opening of the hanger near the glans. Right now it sounds like you’re hanging straight toe but most guys hang toe out. Toe out means that the bottom nut (the nut closer to the glans) is wider than the top nut. See here and here. This is going to take pressure off of the glans, just like Piet suggested.
My advice to you would be to take a few days off from hanging until the hematoma fades, adjust your toe settings and go back at it lightly for a little while until you find your stride.
