The tunica is your shaft. The suspensory ligament is what you target when you hang BTC. Where have you had excruciating pain? This isn’t like hitting the gym. No pain no gain doesn’t fit in here. Figure out what is hurting you and fix that. If you have injured yourself, take a break to heal. You only have one dick. If you ruin it, that is what you get to deal with for the rest of your life.
