Was the head cold? If so, you may have the wrap to tight, limiting the circulation. As long as the wrap is loose enough, you can tighten the hanger a lot.
If you are just talking about the skin under the wrap, it probably is no big deal. But it begs the question, are you using a hot wrap when hanging?
Also, you did not mention the color. If the color is normal to slightly dark, it is probably fine. If white or blue, that is bad and you are cutting off circulation.

I might not understand the question, but know this; you will have to stretch skin first. For the vast majority of guys, skin is the first limiting factor. But it does not take long or usually much effort to stretch and grow.
When it had stretched a certain amount the internal structures will begin to take the load. If you pay attention and learn to tell what is going on, you will know when this happens.
As far as skin stretch, there is little if any boundary as to how much or far skin can stretch. Just look at some of these people that weight a thousand pounds. Their skin stretches an unbelievable amount.
