With the Penismaster, as you wind up the tension, you can feel the ligaments tighten, and your glan starts to hurt (the part wedged ontop of the distal edge of the comfort strap). If your wrapping is not tight enough or you strapped in without enlarging the glan with blood, then your glan is softer and tends to deform and try to slip itself back through the comfort strap, which eases the tension so is no good.
Usually, if I stretch at close to maximum tension (0 - 1 bars visible on the spring grading), no matter how well I wrap myself or how big I make my glan before I strap in, I start to experience slippage when I go past the 30-40 minute mark, which is not too bad, since I have to rest at 50-60 intervals anyway. But sometimes it hurts when your glan tries to squish itself back through the comfort strap, and you have to unstrap and set it up all over again.
With the Vacext, you can also feel the ligaments tighten, but I don’t get pain at the glan, due to it being cushioned by the entire constriction sleeve and the vacuum pulling. At very high tension, part of the glan comes out of the sleeve, but there is no pain and the glan doesn’t fully pullout, the vacuum sleeves are still working so its fine, just crank up the tension a bit more and am back to max stretch.
Every individual is different, but it seems like we are supposed to be at a tension which we can feel a good stretch in our tunica or ligaments, but not painful. If it hurts then you’re going too far. For me, that level seems to be roughly at when my flaccid is stretched to the same or slightly longer length than my erect length.
