My mid-shaft girth is around 5.5” (with a base girth around 5.75”). Magnums are fairly snug on me.
If you pull a condom on tight and leave no room at the end for ejaculate, the condom is likely to break. That’s often why they break, more so than being too big for them, or having a faulty condom. The other reason they break is people use oil-based lubes and latex becomes weakened when it contacts oil - even trace amounts of oil that have been wiped off.
If they are breaking when he comes, then I’d say he’s pulling them on too tight and not allowing room for the ejaculate.

No. Not exactly.
If you don’t have proper lubrication (from the woman or elsewhere), high intensity friction can cause it to break. But that is not dependant on the use of lube inside the condom.
It’s not a bad suggestion to use extra lube (providing that it’s water based), it can’t hurt, but I doubt it’s the reason they’re breaking.
I’d bet it’s operator error.
