The Kinsey survey used an index card of some kind. So while it was probably not bone pressed as you would with a ruler, the index card, being very flexible, could curl up the abdomen a bit, I would imagine. We will never really know, being it was a self-measured survey, though.

The Kinsey Institute seems like the obvious place but you have to go there physically, they have some material online but you have to join and have a good reason for joining.
