Help me understand high testosterone

I’ve been on TRT for a few years now. I’ve had some really great moments, and then I have periods of time like what I have had for the last 2 months where I question the point of doing it.

Current symptoms/observations

- More fatigue
- Weak erections
- Very low libido
- Not resting well
- Flaccid looks pathetic (I have never had a massive flaccid hang, but I look near dead and shriveled up most times)

I had labs run on 12/27 and my T levels were high (over 1,000 with free over 21). Estrogen was normal range (24). They are wanting to decrease my T and leave everything else the same for 4 weeks and then see how I feel.

Granted my levels are slightly elevated, but would that level of elevation cause symptoms that look like my T has bottomed out? I’m very concerned about my erection levels, overall enjoyment of sex, and why my penis looks like it is on life support.

Start: BPEL: 5.25" EG: ~4.5"

Current: BPEL: ~ 6.75" MSEG: ~4.75" BG: 4.825" BPFSL: 7"

Initial Goal: NBPEL: 7" EG: 5" Long Term Goal: 7.5" x 5.25-5.50"

So how’s your thyroid doing? By the way what kind of doctor is treating you?

Post complete labs and I’ll let you know what I think.

1) Are you better off than you were before you started trt?

2)Maybe do a 24 hour urine?

3)Are you doing the sensitive estrogen or the regular?

4)Is your doc an expert or sorta an expert?

Started 6.4" in the tube 2/18. Now 8" 6/10/19 in tube. 6/10/19 BPEL 7.7", BPFSL 8". Began extender 9/4/18, followed by pumping. Short term goal 8" in the tube.Long term goal. 8" x 6" outside of tube. Penimaster rods 11/6/18: UP: 2x.5cm, 2x2cm, 2x4cm, 2x8cm,2x6cm

To answer every question in one post:

Age: 38
Total T: 1026.7 ng/dL
Free T: 21.97 ng/dL
Estradiol: 24.5 pg/mL

Thyroid will not be tested again until May. Last check it was fine.

Am I better than when I started? If you go by lab numbers, yes. If you go by how I feel, I am currently about the same. I’ve had times where I’m great though (high energy, sleeping well, raging libido, great erections, great flaccid, etc). Right now is not one of those times.

The clinic I go to specializes in men’s health, in particular, testosterone treatment. The biggest negative to it right now is I am rarely face to face with anyone other than a PA (and I don’t particularly care for the one that is there now). Some of the PA’s in the past were great, listened well, made adjustments as needed. This one really needs to be replaced.

I do see a doctor there when he is in, though. And the PA’s work is typically reviewed by the doctor’s.

The PA seems to think that if my T is too high that it may mimmic some low T symptoms. I have never heard of that. They did bring my dosage down and said we’ll retest in 4 weeks if nothing feels better.

As far as 24 hour urine or sensitive E testing, no idea about those.

There are many cases where high T mimics low T. A visit to a good endocrinologist might help. Sometimes these mens’ health clinics are all show and advertising.

First of all what is your age?
Second your test 1000 is way to high which is causing you to have a limp dick. Your natural test has shut down because your putting synthetic test in your body telling your brain to close down your Free test.

Your total test is what to high. How many MG of test are you doing a week and how many shots?

I know a lot about this I been in the body building bizz for many many years.

Age 38.

One shot a week. They just dropped my dosage from 130 mg/dL to 120 mg/dL this week. They also give me a shot of HCG each week.

Was anything else tested? Specifically looking for DHT and Prolactin.

Do you have a copy of the lab report? The Estradiol test should state which methodology was used. It’ll be either immunoassay (ECLIA) or mass spectrometry (LC/MS/MS, sensitive). This makes a huge difference in interpreting the results.

Sorry, but this is nonsense.

120 mg is too mcu test at your age. I’m 46 and only do 50mg twice a week to keep my levels normal.

If you continue down this path you going to be in for a world of hurt as you can see you’re starting to suffer already.

Before taking test do you normally have Low T to begin with?

Says “Roche ECLIA methodology” on Estradiol.

Nothing else was checked except blood levels (to makes sure blood was not thickening). We are looking at testing Fe and B12 levels on next labs if symptoms persists.

My original test ranges put me around 350 total, but I cannot remember the free (I just remember it was below normal range). They have had issues over the last 12 months keeping my hormone from having large fluctuations. In 2017 I was fairly steady and they were decreasing T regularly. In 2018, I’d go from high, to low, to high, etc. Keeping estrogen in check has been another thing they have just figured out with me. What was crazy is I lost more fat and weight in 2018, yet they could not keep it in line.

350 is not bad at all do yourself a favor and lower your dosage or come off all together for quite some time to fix everything.

I used to be into body building my friends call me Mr scientist when it comes to all this stuff.

I put people on cycles all the time and post cycles.

Come of and let your body heal for a good several months or drop it down to 60 mg once a week

Your doctor doesn’t even know enough to order the correct blood test. Find another physician ASAP.
